Donation Request Information

With the outbreak of COVID-19 virus, resources such as personal protective equipment, disinfectants, wipes, gloves, etc. that protect individuals from the virus have become increasing more difficult to acquire. Given how crucial these resources are to upholding the safety of an organizations staff and clients, CHHRGE wishes to support its members by helping supply these resources or connecting organizations with other non-profit organizations who can meet their need. 

Below you can fill out a Donation Request Form below to request supplies you may need support in acquiring. Once a form has been filled out, a member of CHHRGE administration will reach out via email to discuss and clarify how they can support you in acquiring the supplies you require.
